Getting help early can make all the difference.
The Care Coordination program works in partnership with Mob Link to support Aboriginal and Torres Strait Islander young people and their families before things get too tough.
This early intervention program connects young people with culturally safe services and support through ATSICHS Brisbane.
How does our service work?
A dedicated Care Coordinator works directly with young people and families to:
- Understand what support is needed
- Link them with the right programs and services
- Coordinate care across the ATSICHS Brisbane Community Services team
- Make sure support continues with follow-ups when needed
The goal is to make it easier to access the right help at the right time.
Support may include:
- One-on-one assessments and referrals
- Help accessing ATSICHS Brisbane programs like youth services, social health or family support
- Guidance for workers and families needing culturally appropriate pathways
- Coordinated care and ongoing follow-up
